Therapy/Rehabilitation Specialty: Physical Therapy Home Health
Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 -
17:00) Experience: Minimum of 2 years required License: Current
state physical therapy license Certifications: Basic Life Support
(BLS) Must-Have: Home health experience Description: CA license and
certifications must be in hand to submit. Candidates must have a
minimum of 2 years of experience. Home health experience is
required. The area of coverage includes San Jose, South Bay, and
Santa Clara County. Specific charting experience is required, with
preference for Epic experience. Background checks will be performed
prior to submission. A 7-year work history verification is required
for all candidates. Candidates must provide accurate work history
information going back 7 years from their start date. Documentation
or contacts for verifications may be required if positions cannot
be verified. References obtained within the last year are required.
Candidates should include their former manager’s name during
submission. Locals are accepted at a local rate, and
