Registered Nurse (RN) is responsible for coordinating and managing
patient care across the continuum to ensure high-quality,
cost-effective outcomes. This role involves collaboration with
interdisciplinary teams, patients, and families to facilitate
efficient transitions of care and promote optimal recovery. Key
Responsibilities: Assess, plan, implement, and evaluate
individualized care plans for assigned patients. Coordinate patient
discharge planning and post-acute care arrangements. Work
collaboratively with physicians, nurses, and other healthcare
professionals to ensure continuity of care. Monitor utilization of
healthcare resources to promote efficiency and cost-effectiveness.
Serve as a patient advocate by supporting informed decision-making
and ensuring access to necessary services. Maintain documentation
