Role Description This is a full-time on-site role for a Hospice Liaison located in San Ramon, CA. The Hospice Liaison will be responsible for building and maintaining relationships with patients, families, and healthcare providers to ensure access to quality hospice care. Responsibilities include educating patients and families about hospice services, coordinating referrals, and facilitating the enrollment process. The role also involves supporting care teams and community partners in understanding and promoting hospice and palliative care services to enhance patient outcomes. 90k-150k plus bonuses Qualifications Expertise in Hospice Care, End-of-Life Care, and Palliative Care Proficiency in Sales and relationship-building within healthcare settings Experience or familiarity with Home Care services Strong interpersonal and communication skills Ability to work collaboratively with healthcare teams and community partners Knowledge of healthcare regulations and standards, particularly within the hospice care context Bachelor’s degree in healthcare administration, nursing, or a related field is preferred Previous experience in hospice or palliative care liaisons at least 3 years must have strong book of business
